The problem
Clinical staff needed answers from long internal documents, and an answer without a traceable source was not usable.
What I built
An ingestion pipeline and a retrieval agent that combines dense and keyword search, reranks the results and verifies each answer against the retrieved passages before it is shown.
Key decisions
Hybrid retrieval with reranking
Dense and sparse search together catch both meaning and exact terms.
Answer, then verify
A second pass checks each claim against its source and drops what it cannot support.
Cite or stay silent
If the documents do not contain the answer, the system says so.
